Go to the leader boards, and there is a tiny little phantom skull in the bottom left corner of the page. Click on it, and you will get a bunch of test. When it asks for the passcode you type it in and hit enter. Boom! Unlocked.

Just know you have to unlock Phantom Prospect and Phantom Trainee first...and that is done the same way.
